Midfoot Injuries (Lisfranc Injuries): Lisfranc Reduction With a Plate and Screws Animation

This animated video will provide a demonstration of a surgical treatment using a plate and screws for a Lisfranc injury. In review, the anatomy of the Lisfranc complex includes 4 bones: the medial cuneiform, the intermediate cuneiform, the second metatarsal base, and the first metatarsal base. The complex also includes several ligaments, including several pictured here on the top of the foot. When a Lisfranc complex injury occurs, damage to these ligaments and widening is seen between the bones.

In this procedure, a plate and screws will be used to bring the bones back in alignment. Here is the top of the right foot. The surgeon has already made an incision to the top of the foot to expose the complex. An instrument is used to reduce the bones back to their proper alignment. The plate will then be placed in the incision, overlying the complex, and held in place with 2 temporary ball-tipped wires.

Although not shown, holes will be drilled into the bone and measured before screws are placed in each of the holes. As the screws are inserted in the holes, the joint space nicely reduces into place. The temporary wires are removed, and the incision will be closed. The surgery is now complete.
